I want to remind everyone that this is only an approximate size. I have been looking over several different sizers and some are slightly different. This one seems to be the most accurate of all the ones I've looked at. My advise is to have him measure from start to finish using a piece of paper then get your estimate from one of the online sizers. After you have an idea then take your paper to your local jewelry store and have them double check it against their store sizers.
Also, if he knows someone who happens to know their ring size he can try on their ring to see if it fits. Just another way to double check the size before you buy/order his ring. Be sure to allow yourself plenty of time to return or have it resized.
