I just got done posting the SRCI Current for Volume 11, Issue 32, August 7, 2008 Newsletter. On page two of it, there is a Notice of Survey for Health Services. Everyone with a loved one at SRCI should please please take a look at this. Then, the contact info is posted in it if you want to respond.

Thank you for posting this information! Our son is dealing with a health issue, has repeatedly requested his medical files, HIPPA waivers etc to no end. This gives us an additional avenue to pursue!
