Do you need to fill out a form if he goes back to prison a few months after getting released???????

If he paroled and was returned to prison, you need to have him send you a new visiting form, you are no longer approved until he does.

Yes, your best bet is to send in another application. However, he can find out if you are still approved. They say you could have to reapply after 2 years. my brother is still approved after 10 years and 3 terms later. go figure!
