Default Free online printable living will.

Here is a link for an online printable living will. None of the info. you provide is recorded. The questions are for printing purposes. You will have to fill it out, and get it notarized. I printed one out for everyone in my family. It is not questions like the living wills you would get from a hospital. This is only for providing nutrition, or not .I think everyone should at least have one of these. It is free.
There are more free ones out there. Just do a search for free living will.

Please make sure that this will work in your state before counting on it to work for you. Some states have their own special forms and others may not even recognize the forum that you find on the internet.

I did this and found one specific to my state. They have a list of all 50 states. The site is www.legaldocs.com

Thanks cjjack, I printed one from that site.
Also I wonder if I could send one to my guy in prison? Or would the prison not allow it because he is in their custody, and they will do what they want with him, if something were to happen to him?

I Living Will only makes your wishes known--it does not mean they will be honored. You also need to get a POA for Healthcare, and designate someone who you know will honor your wishes.

The form I downloaded has a place where you put your primary care physician, and it is up to you to make your wishes known to him or her.

Your Dr may know your wishes, but he is not your appointed guardian. You need to have someone who will honor your wishes. Otherwise, whoever is considered your "next of kin"
gets to decide your fate. They could demand that you stay on a respirator, feeding tube, etc.
