I also faxed mine, and priority mailed a hard copy. Cost about $9 in total (from NY to FL) I did that on a Monday and was approved yesterday so it took about 10 days total. I also called his CO and VERY nicely asked if it had been received.

Some facilities will not accept a fax copy at all so it's best to call to see what procedure is at the facility where your loved one is.

Are the forms in this thread still up to date? Should I go ahead and send one in, or wait until I receive one from him to complete? He's still in reception, but I want to see him as soon as he is allowed visitors!

I just used this one in May. I overnighted the original and faxed one as well as others have mentioned doing the same and it helped get the process started. You should get your Vinelink notification the day he transfers so you'll know where he went. Give it a week or so, and you can call to check the status. I waited a week and a half and it had been approved the day I called.

Default visitaion forms?

My baby is in cfrc-e and I wanted to know if there is a visitaion form I can fill out and send instead of waiting for him to reply to tell me what to do. Thanks!

does anyone hv a copy of the visitation form for SFRC??? I had one mailed to me but they didnt get it so I wanted to save some time and see if I could download one and was unsuccessful Please lemme know if you can help!!!! thanks soooooo much Merry Christmas

Does anyone have a copy of the visitation form? Mines got burned up with my office with 2 other rooms. It was my ex boyfriends last one. He's always been a best friend to me and I finally had the nerve to go and then my dryer caught on fire. Just my luck. Anyone can mail it to me to make copies? Or know where I can get one?

DOC doesn't make the form publicly available via its website or other means. The only way they distribute it is by giving it to inmates to send to their people, to control access to the form. So unless someone else posts it online, like in this forum, there is no place to get it.

Look what I found on my hard drive! My husband laughs at my digital hoarding but it comes in dang useful sometimes!

The pdf is useful for people like me on Macs who can't open .doc files without it completely hosing the formatting on them. I had to have my office staff make it for me!

Thanks so much for the form. I am mailing mine tomorrow. Hopefully it won't take to long to get approved. Been through this before so there should not be much delay as I am already in their system from before. Missing my man so much.

Thanks so much for the form downloads, have been searching for a week. seems every time a letter comes someone is giving my girl mis-information about the visitation form where you get it and where you send it etc. same with the phone number approval. not even sure if her location is temp or perm, hard getting good info, that is why I signed up for prisontalk, seems the best location for finding everything I have needed so far.

I just wanted to say thank you again for this download. Within 5 days I was approved for visitation. My man was not even in the system 3 weeks and we were already having our 1st visit. Of course, I must say that I did not wait for him to notify me. I contacted the facility myself to see if I was approved. If I had only thought to to call them on Friday instead of Monday we could have visited him sooner. It was wonderful to get to spend 2 days with him.

Per the DOC, when you are done filling out the form, send it directly to the camp / prison where your loved one is; address it to "CLASSIFICATION DEPARTMENT" unless you know who your loved one's classification officer is, then send it directly to that person.
