Had the same experience but not on the scale you did. Procedures are only as good as the people who follow them. Makes me wonder if these people even know what is contained in the procedure of have even read it. I also wonder who has access to these signed forms and how quickly they can get to them in case of an emergency. It shouldn't take days of calling to get an answer to a question in this type of matter. Perhaps this needs to be brought to the attention of those higher up perhaps these people are unaware this system of notification is broken and then they might work to make this procedure go more smoothly and make sure the people involved in this know what they are supposed to be doing and will be held accountable.
