Awaiting-trial prisoners at Pollsmoor Prison in the Western Cape regularly swap identities, leading to the mistaken release of dozens of inmates facing serious charges including murder.
Awaiting-trial prisoners carry cards bearing their full names, cell numbers and thumbprints, but not their photographs.
