There is a form that needs to be filled out in order to for the 15 year old to be able to go in with someone other than her parents and the form has to be notarized. I know because I notarized this form for my cousin. I don't recall where she got the form from. For correct and accurate information, I would call the prison and ask.

Here at PBSP CA......You would need her birth certificate and a written letter from her parents that it is ok to visit her friend, with his mom.Good Luck....Call prison also to find out information.

I believe most prisons have their own form you can fill out and have notarized to bring a minor to visit who is not with their legal guardian. Back in '97 my husband was in CCI and I just typed out my own permission letter for my Mom to bring my kids into visit and I had that notarized. I think your best bet is to call the prison first and find out if you can write your own or have to use one of their forms.
