The form asks for my identification card and issuing state!! What does that mean...I'm Canadian and I don't think we have identification cards. I filled out the drivers license part...is that enough ?? Or what exactly is an id card ?

its asking for your state issued ID number adn the state that issued to to you. thats all. for instance mine is from california adn id just put down the number. if you have a driver license you wont need to put down an id number....
