View Full Version : ASPC-Yuma, Visitation application, I need Info!
Golden Bough 02-25-2004, 11:15 PM I have a Nephew that has just been sent to ASPC-Yuma. He is in the process of filling out the Visitation Paperwork and phone call list. Before he arrived there, he was in Towers Jail, Phoenix where I sent him a copy of ADC's 911.01 Visitation Application Process. I also sent him all the required information that he would need to fill it out, i.e: my full name, address, DOB etc. He just wrote to me and told me that in order for him to complete his part of the form, he needed my Social Security number. SAY WHAT! That would be the last thing I would send him! The information I originally sent him said nothing about the that. Since I have yet to see this application form, I have no way of knowing what I will need to provide. I don't doubt that it does ask for it, but I do doubtthat the inmate is the one who is suppose to provide it. Can anybody please tell me if the Visitor's part of the Application requires SS #'s ? I want to believe that he is confused about what he needs to fill out and not trying another one of his scams again. Myself and his Grandfather are the only blood that still care about him, I want to believe in him.

Well, for what it's worth since I deal with the state of Alabama and not Arizona - but Alabama inmates are always the ones responsible for filling out all info (including SSN) on visitation, phone, and funds lists. Matter of fact I have never had to fill out anything at all for anything, he always has to do it. So what your nephew is asking is not unlikely at all... but hopefully some Arizona folk will be able to come along in here and confirm the answer to your question soon.
That said, since you had some specific visitation questions for an Arizona prison, I have moved this thread to the Arizona forum on PTO so your post will have the opportunity to get more specific responses and the best answers to your questions, and won’t get lost in the busy Visitation forum... I’ve found they often get more and better answers there, AND the information will be here to help others in the future, too. :)

JoeysGirl 02-26-2004, 04:06 AM Okay...I can help you out on the visitation process. My fiance was in Towers too, and now just got sent to Florence. What I believe, is that the inmate puts you on their list for calls and visits, and then they send you the forms to fill out, and then you fill it out and send it back to the Visitation Department in his prison. They do not accept the forms if you fill it out and send it to the inmate.
I have a visitation form, right here in front of me....Here are some of the basics that are on the form and also an answer to your SS# Question. (I believe they are the same for all ADC Prisons)
Inmate Name/ADC #
Institution/Facility.......Question about Accepting Collect Calls or not
Requested Visitors Name/Age/Telephone #
Then it says: The requested inmate above has requested that you be added to his/her visiting list.If you want to visit this inmate,please complete the following items. Then it talks about filling out the form for a child.
The next section is the ADULT Part.
Name....Other ALiases or names used
Residental Address/City/State/Zip
Mailing Address/City/State/Zip
Date of Birth/Weight/Height/Eye Color/Sex/Ethnicity
Drivers Licence Number or other Photo ID Number/**Social Security#
**** IT says."Disclosure of your SS# is Voluntary. IT is being requested pursuant to ARS 41-1604 (A)(2) and will be used to verify identity and background info necessary to determine your suitability as a visitor to an Arizona Dept"
Then the back part just has some basic questions about if your a victim of a crime by this inamte. If your on any other inmates visitation list......stuff like that.......

You do not need to put it on to be approved. They can not deny you because you did not give it. I am Canadian so I do not have one and I had no troubles being approved. Just tell him to skip that question.
WINDENSGRL 03-10-2004, 01:17 PM They CAN deny you for any reason, especially YUMA cause they are notorious for making it as hard as possible on you and your loved one. They are also notorious for making up their own rules and policies. They sent mine back cause I put my zip code on there with one digit backwards, so don't be fooled! It took me almost 60 days to get approved!

BabyGirlStacey 11-15-2004, 02:55 AM My man is in Cheyenne Unit-Yuma. He said when he got there that he needed mine and my daughter's SSN. But we were denied anyways so I guess it doesn't matter, but I do know on the app, it says "voluntary".
MissingMyGuy 11-15-2004, 08:49 PM This is really strange.... when MyGuy 1st went in he was in Tucson ... He had to mail me the application, I had to fill it out, and return it to the visition office. He understood that there was no other options. :confused:
MyGuy is currently at Cheyenne Unit CDU ~ and YES :eek: ... they make it as absolute difficult as possible for friends/loved ones. They just recently changed visitation days from Monday and Friday to only Tuesdays. Needless to say .. they had plenty of visitors on the other two days, and now we only get ONE day .. and if you don't schedule far enough in advance ... you don't get in!!
Thank goodness we don't have much longer to go until our nightmare is over!! :p
