I filled out the visitation request form for USP McCreary last week and mailed, but after reading some other posts I have a concern. I listed my office address on the form because I do not like my home address floating around the prison. This is where he sends his letters to me, but it doesn't match my drivers license. I don't think that will have any bearing on you visiting. I sent mine in and have a PO box as my mailing address and that is what I used and that address was also different than what is on my DL. Your loved one's counselor will notify him when your visitation is approved and he will let you know. The inmates never see the forms because you mail them back to the counselor. It took me about 2 weeks to get approved no problems. Have a wonderful visit. I have a post under USP Mcreary information and it has some useful info about the visitation, hotels etc. hope it helps. P.S. If for some reason you do get denied and you think that is the reason (although I doubt you will) you can write to his counselor and explain why you use your work address but I don't think it will be a problem.
