I think there is one on the ADC website that you can download. I ran across one a few weeks ago when i was doing a search on DOC procedures re inmates accounts.

You can call Trust Fund Centralized Banking at 870-850-8479 or email them at banking@mail.state.ar.us and maybe they can answer your questions or email you one.
