You can get a form from OKDOC website but it is best if the inmate mails you the form from the facility-I'm not sure whether or not the facility forms vary or if they all use the State one. Once you get the form, you'll need to fill it all out and mail it into his case manager for turning in for approval. the approval could take 2 weeks or 2 months, never know it varies by facility and staff on hand. Sorry this isn't much help.
