This is the link to the DOC site that has the release inmates will have to complete in order for DOC to respond to any inquiries. Go to the bottom of the page and print DOC 060212F It opens in Adobe.

Also as a reminder, here is the link to Medical releases

Go to the bottom of the page and open DOC 140108A for authorization for release of regular health info

And DOC 140108G for release of Psychiatric/Psychological records

The person on the inside my not have any problems now, but I would recommend that you get them completed now, just in case they are needed sometime in the future.

I'm not sure I understand the significance of these.

Are they to withhold info on inmates, due to privacy issues? Or are they to release info? If so, to whom? Private medical facilities? Family doctors?

Why must they be completed now? I'm sure if I go to the links and read all the criteria, I might have some hints...

But, being a lazy slob, I'm hoping you can enlighten me much more quickly, and IN ENGLISH!

THEY ARE RELEASES SO THE PRISONS CAN RELEASE INFORMATION TO SIGNIFICAN OTHERS- I THINK THESE ARE FROM OKLAHOMA BUT THERE ARE RELEASES IN EVERY STATE-

WOULD BE HELPFUL TO HAVE IF PRISONS EVER GIVE YOU FLACK ABOUT GETTING INFORMATION ABOUT YOUR LOVED ONE

I have gotten a General Power of Attorney and had my husband/brother-in-law fill them out, had them notorized and sent back to me. I then went down to the court house and had them registered there and then sent them to everyone concerned in the prison system where they are at to be put in their files so that if I were to write or call about them they can't say I don't have authorization.

I found out about this when my brother-in-law needed help when insulin was being refused to him. I was told he needed to sign a release authorizing me to the information. I told him, he said he signed and they said he never did. So, this took care of the authorization on this end.

This is in Texas that I have been using this and with INS.

I'm thinking of talking to Billy about power of attorney. He's having some medical problems and the prison won't tell me anything because I'm not his wife. Maybe if Billy agrees to it then I will have some say so out here when the time comes.

Now my question is .... my fiance' just filled out power of attorney papers for me to be his poa. would that work to get info or am i going to have to fill out release papers for them to give me information?

Does anyone know if I would have to fill out those papers for my fiance' due that I have become his power of attorney while he is incarcerated? He is in state prison in washington state...
