Hi All,
First I am so glad to be able to have someplace to go to ask for help.:D
Okay, I received the visiting form today to fill out.. it is all easy except for one thing.
Form question 11. Have you ever visited another inmate(s) in a California Prison? If yes Inmate Name, CDC Number, Institution where you visit inmate, relationship to inmate.

Here is the situation: 19 years ago, a close friend was in a very bad car accident he had been sort of racing another car (wasn't a formal race or planned and still dumb) and his car hit a minivan that turned in front of them, sadly people died. He plead guilty. Really sad for all.. :( anyway before he was formally sentenced, the judge sent him to Vacaville for evaluation or something like that. I went to see him there. I remember having to be approved but it wasn't like this where they send you a form you fill it out and return it. Anyway, he was only there 30days or something like and I think I only went once to see him and then when he was released back to county the judge sentenced him to a work furlough program. I can't call my old friend because I lost contact with him and I don't remember if he had a CDC or not. (Oh and last I heard he was doing really well, no trouble, no racing... does some sort of rodeo thing)

Okay, I will get to the point... what do I put on the form?

I want to send this form in ASAP so any suggestion would be appreciated. I did try and call the Vacaville prison but couldn't reach anyone. Tried calling NKSP but of course I don't know the extension and the attendant doesn't answer.

Hi lostintheweb if it's been that long then don't put anything.I went to visit my DoggyDog 4years ago when he was in prison, and now that my husband is incarcerated, I too had to fill out the same visiting form,and I just put does not apply. And everything went smooth.:yes:I think they just want to make sure your not a red flag or something.:eek:

If you have ever visited an inmate in a California Prison you must list their name on the app. Even if it has been 19 years.

Hi, thanks. I actually did put his name and then just put a question mark in the CDC number spot with a 1986? Sent the form this morning let's keep our fingers crossed. Thanks all for the feedback!
