You don't have to fill out a new form. But at Centinela they do require that every two years you fill out a new form

Hmmm. When my friend went from Avenal to CMC, I had to fill out a new form. At least that is my recollection...

When my husband was transferred to Solano, they tried to tell me that I couldn't visit because I wasn't in their computer. I had checked the CDC website and their visiting guidelines state that your approval will transfer from prison to prison. I took my approval paperwork with me, but they still wouldn't let me in. I asked for the Visiting Sgt. and he didn't want to let me in either. I told the first officer, "Your visiting guidelines state that my approval transfers from prison to prison." She said, "I don't know anything about any visiting guidelines. If you're not in the system you can't visit." I said, "You don't know about your visiting guidelines? It's on the department's website and signed by your Director." It wasn't until I told them I'd like to file a formal complaint that the Sgt. took my license and went into the other room. He came out and said, "Oh, there was a minor mishap. There was erroneous information on your record." I thought to myself, "Just a minute ago you told me I'm not in your system; now you not only have my record, but there's now erroneous information on it?" Needless to say, the minute I brought up the word 'complaint', it was all taken care of and I went right in. I did have to fill out a new form a few months ago, however, because it had been 2 yrs since my last form. They need a new form every 2 yrs. Good luck!

I also know that you do not need to fill out a new form, actually, there should be no need for you to fill out a new form, but who knows, sometimes CDC just gets a kick out of making our visits imposible, but good luck!

Thanks for all the info it is all appreciated. But my husband sent me a copy of his 120 card and if I'm on it I don't have to fill out another form.
